Medical Forms

“A health history revised within the past 12 months and a physical examination
conducted within the past 36 months are required of each camper and adult under
40 years old. A copy of the physical form must be attached to the health
history. A parent or guardian must attest to the validity of the health history
and physical examination for those under 18 years old. Adults over 40 years old
must show evidence of physical examination with in the past 12 months.” Any
person, youth or adult, staying at the resident camp and arriving without a
completed health and medical record will be sent to the nearest doctor to have
an examination at his or her own expense. If no exam is available, she or he
will be sent home. All health forms require a doctor and parent’s signature!
Youth and adults under the age of 40 should complete the health history and physical examination on form #34414 (Class 1 and 2). Adults over the age of 40 must complete form #34412 (Class 3). Both forms require completion of health history and physical examination. We will only accept non-BSA physical forms if they are attached to the complete health histories on a Class 1 and 2 or Class 3 medical form. Scouts participating in High Adventure programs (C.O.P.E., Climbing), will need to complete the Class 3 form.
