“A health history revised within the past 12 months and physical examination conducted within the past 36 months are required for each youth camper and adult under 40 years of age. A parent or guardian must sign to the validity of the health history and physical examination for those under 18 years of age. Adults 40 years of age and over must show evidence of physical examination within the past 12 months.”

"Use form No.34414 (Class II [pdf file]) for health histories and physical examinations of youth and adults under 40 years of age, and form No. 34412 (Class III [pdf file]) for adults 40 years of age or over. Both require complete personal health histories and examinations”.

Any person, youth or adult, staying at Webelos Camp and arriving without a completed health form and medical record will be sent to the nearest doctor to have an examination at his or her own expense. If an exam is not available, he or she will be sent home. All health forms require a doctor and parent’s signature!
